January 2012—Every month, a hypothetical management situation is posted. You are encouraged to share your thoughts (in the comment box below) on how you would address the issue. Here is this month’s question:

Despite your best efforts to prevent a situation like this, one of your patients was overexposed to radiation during a CT scan. What would you do, and how are you working to prevent this?

  1. Root cause analysis followed by education. A physics consult would also be appropriate.
